Abstract:

BACKGROUND
The American Association of Nurse Anesthesiology (AANA) identifies transfer of care as the eleventh standard of anesthesia practice, emphasizing the CRNA’s responsibility to ensure continuity of care. Literature supports the use of standardized handoff tools to reduce errors, improve patient safety, and strengthen communication.

PURPOSE
This quality improvement project was part of a multi-center study within the Advocate Health System in Charlotte, North Carolina. The study explored similarities and differences in how CRNAs perceive and use standardized handoff tools across facilities with varying patient loads and acuity, including a Level I trauma center, a suburban adult acute care hospital, and several rural hospitals.

METHOD
This project took place at an adult acute care hospital. A 15-question survey evaluated CRNAs’ perspectives on standardized handoff tools, including usage patterns, preferred communication methods, and perceived barriers. The survey also asked why tools were not used and what improvements might increase adoption.

RESULTS
CRNAs reported patient safety as the main benefit of handoff tools. Barriers included workplace culture, time limitations, and viewing the tool as an added task.

CONCLUSION
This project supports using concise, efficient handoff tools to promote consistent, standardized communication in clinical practice.
